Our team of specialist and generalist nurses provide nursing care and treatment for people with complex, long-term conditions through to one off service in their own homes.
Our community nursing teams support house-bound people with a wide range of mental and physical health needs, from wound management and diabetes to kidney failure and heart or lung conditions.
We work closely with GPs, hospital staff, consultant specialists and other health professionals to identify a person’s individual needs and help to co-ordinate the different services they receive.
Our district nurses also provide information and advice to help patients (and carers) to manage their health confidently and independently at home. This helps to avoid complications associated with immobility, disability or existing illness, and to minimise hospital admissions as much as possible.
We’ll work with your GP to provide you with your personalised treatment plan and follow-up programme.
