Our Post Discharge Support enables care at home for frail elderly of New Zealand who would otherwise be re-admitted to hospital or residential care.

Through the Canterbury District Health Board Community Rehabilitation Enablement and Support Team (CREST) programme we support the frail elderly of Canterbury who would otherwise be re-admitted to hospital or residential care. 

CREST is the result of a strong collaboration across primary (General Practitioners) and secondary (Nursing, Specialists and Allied Health providers) care and its continued success owes much to the sharing of patient information to provide a seamless patient health record. Access to CREST is through your GP.

Reablement Services

Reablement services are offered through Counties Manukau District Health Board. This service is provided for up to six weeks in your home usually after a period of illness. It is aimed to help you regain your confidence to live independently.

A member of the Reablement team form the DHB will visit you to develop a plan around your individual needs, goals and recovery. Then an Access Support Worker who has been trained in Reablement will assist you to getting back to being as good as you can be by following the plan. They may visit you up to four times daily if required.

If one of your care team in the hospital offer you Reablement services for when you go home then you can ask for Access Community Health to provide the service . They will then organise this for you.
