Care Coordination Team Leader


Porirua, Wellington

The Care Coordination Team Leader contributes to the delivery of our home-based support service by:

  • Working as part of the regional leadership team to deliver quality home and community support services to our clients
  • Facilitating co-operative and cohesive working relationships between our Nursing, Care Coordination and Support Worker teams
  • Leading a team of 14 care coordinators to facilitate the delivery of a responsive client focussed home support service
    • Ensuring our support workers are rostered safely and effectively to meet the needs of our clients
    • Overseeing all new referreals to ensure effective rosters are put in place
    • Championing customer service and communication excellence
    • Managing escalations and complaints

We are looking for someone who shares our passion for supporting people to remain active within their communities.

Requirements are:

  • Customer service champion
  • Experience in leading a large busy team
  • Excellent interpersonal skills
  • Experience in logistics, planning and prioritisation
  • Excellent written and oral communication skills
  • Thrives in a fast paced environment
  • Results driven achiever
  • Strong team player who is able to work effectively with a wide range of people
  • Excellent computer skills.
