We work together to help you to improve and restore function, building your strength, capacity and independence. This can include coaching of an exercise programme designed to increase mobility or prevent falls, assistance in re-establishing social connections in the community, regaining skills and practical support to keeping the whole household going.Restorative & goals-based support includes:
- Restorative care â examples of how we can help include encouraging safe use of mobility aids, coaching safe transfer methods, coaching of exercise programmes designed to increase mobility or prevent falls, assistance in re-establishing social connections in the community, regaining skills, mobility and confidence and practical support to keep the household going.
- Allied health services â physiotherapy, occupational therapy, speech therapy and social therapy (including assistance to keep active in the community).
Developing your Support Plan:
- We start with clinical and social assessment using Accessâ own goals-based model.
- We work alongside other health agencies to produce a Support Plan that works towards your goals and maximises your independence and safety.
- We assign a support worker â trained in restorative care and chosen for their ability to meet your needs â who works with you to help you to achieve your goals.

How we support our clients

- We provide you with the support you need to gain control of your life, from helping you to get out and about in your community to keeping you mobile in your own home.
- We focus on setting and achieving goals â goal-setting is incorporated into our standard service model so all of our coordinators are trained in goals-based assessment.
- Weâre experienced in developing and implementing restorative home-based support models (weâve been developing and implementing them for over five years).
