Access provides personalised clinical support, guidance and education to help you manage your long-term health condition. Our modern around-the-clock service is designed to fit within your lifestyle and help you to get on with your life with your health in check.Long-term condition management includes:
- Nursing – continence management, stomal therapy, IV therapy, wound care, catheter care, medication management and oversight, eye drops, symptom monitoring and management information, and condition education.
- Allied health services – physiotherapy – assessment and assistance with mobility; occupational therapy – assessment and aids to improve your everyday living; speech therapy – assessment and assistance with speech and social therapy – assessment and assistance in keeping you active in the community.
- Personal care – dressing/undressing, bathing, grooming, toileting and mobility.
- Household assistance – cleaning, cooking, laundry, shopping and transportation, child care, and helping you to keep in touch with your community.
- In-home respite care

Developing your Support Plan:
- We start by getting to know you and assessing your situation, needs and expectations.
- We draw on the skills and experience of our staff to create a Support Plan that fits with your lifestyle, improves your quality of life, and maximises your independence and safety.
- We assign you a support worker – chosen for their ability to meet your needs – to care for you and support you to achieve your goals. We also provide you with any health and social professionals you need.
