Clinical & recovery services include:
- Nursing – continence management, stomal therapy, IV therapy, wound care, catheter care, medication management and oversight, eye drops, symptom monitoring and management for long-term conditions, and information and education to help you to be as safe and independent as possible.
- Allied health services – physiotherapy, occupational therapy, speech therapy and social therapy.
- Assessment – assessments of your health, mobility, socialisation and how you cope with everyday activities. Plus, specialist assessments such as wound, respiratory, continence, and palliative.
- Discharge and transition planning – organising your move from hospital to your home.
Developing your Support Plan:
- We start by getting to know you and assessing your situation, needs and expectations.
- We draw on the skills and experience of our staff to create a Support Plan to keep you in your home and maximise your independence.
- Our clinical staff work together with other professionals (where necessary) to ensure you receive the care and support you need.

- We give you advice to help you help yourself to recover as quickly as possible, including making the best use of our technology and equipment to aid your recovery.
