It’s easy. All you need to do is get in touch. We’ll do the rest.

All you need to do is contact us and we'll arrange a time to visit you, then put together a tailored Support Plan to meet your needs.

Getting started with Access

Step 1. Get in touch

You can do this in one of five ways:

  • talk to your GP and they can refer you for home-based healthcare, or
  • if you have an ACC contact manager or case manager, ask them about putting you in touch with Access, or
  • contact your local Needs Assessment and Service Coordination (NASC) agency and ask for assistance, or
  • call us directly on 0800 AT HOME (0800 284 663) and we’ll talk about the best way to access the care you need. (If you’re not entitled to funded care through the Ministry of Health, a District Health Board or ACC, we can talk about other options, including private care.).
  • fill out a simple enquiry form and we’ll call you.

Step 2. We’ll visit you

After we’re appointed as your provider, one of our visiting coordinators will visit you at your home to get to know you and assess your situation. You’re welcome to nominate a family member or friend to help you talk with us.

Step 3. We’ll put together a Support Plan

Once we have a good understanding of what your needs are, we’ll work alongside your GP and/or other health agencies to determine the steps to take to help you to maximise your independence. We call the result your ‘support plan’.

Step 4. We’ll appoint a support worker to you

We’ll match you with the support worker who is most suited to meet your needs (and lives within reasonable distance from your home). If required as part of your Support Plan, we’ll also match you with a nurse and/or any health workers you may need.

Step 5. You can live your life to your fullest potential

Your support worker will get to know you, care for you, and work with you to help you plan and achieve your goals. At the same time, you can rely on the support of our nationwide Access network – you can talk to us any time between 6am and 11pm, 7 days a week.
